Totally fine even with heavy tattoos. Everyone's friendly and greets you with "hello" when you come in and "see you" when they leave first.

You can bathe in the onsen just like at a normal sento. The people without tattoos are the ones with the rude, arrogant attitudes.

Apparently it's called a "welfare onsen" because a nearby welfare facility runs it. Old people = higher chance of tattooed folks... and as a result, you can bathe.
