I've checked by phone before and got the answer, "You can get in with no problem," and I actually got to bathe there too. The water is really nice.
I asked at the front desk if it was OK, and I've been back many times since. The water is really nice.

You can get in with no problem. I also checked with the woman at the front desk, who seemed to be the owner, and she said it's fine. It's a nice onsen with a lot of history.
I asked by phone. When I checked with the woman on staff, she said it's fine to just go in as is, without covering up with a cover sticker or anything.
No problem at all. I ended up in the bath with some neighborhood regulars, but they didn't stare at me at all; if anything, we got into a nice chat. It's a compact tub, and there are no showers.

They didn't particularly prohibit tattoos.
